1) Patient Health Questionnaire::
All new patients must e-mail our center the Health Questionnaire so all the medical staff can start getting familiar with your health status and see if you may need a special medical instruction before you get here. You will find the health questionnaire on this website by clicking this link
2) Surgery Date::
After you find a convenience surgery date confirm with us the following:
* Date of surgery: so we can start working on the arrangements for your surgery and get everything ready.
* Also we must be sure that it is an available date.
3) Arrival Information::
Remember to start looking for your airfare as soon as you get a date. Keep in mind that you most arrive at the San Diego International Airport between 9am and 11am, your departure should be on the eight days of surgery after 12 pm.
E-mail us a copy of your itinerary so we can contact you with details on where driver will meet you.
4) Doctors Instructions::
All Patients must be on a Pre-Liquid-Diet, for 10 days before the surgery, we will e-mail you the diet after we receive your $500.00 Dlls Deposit. This way you can start getting familiar with it. Also remember you must come in with an empty stomach, so there is no eating or drinking 10 hrs before your arrival that way we can perform the evaluations with an empty stomach.
5) Payment::
In order to hold a reservation on our surgical schedule, you must make a non-refundable $500.00 deposit in the form of a cashier's check, made out to DANIEL HUACUZ GUIZAR.
This deposit will come off the cost of your procedure. The remaining balance will be due upon arrival, the day of surgery. If you need financial assistance please click here
